The South Thames Paediatric Network (STPN) works across 19 Trusts in the South region covering South London, Kent, Surrey and Sussex. We work with hospitals and staff across the region to ensure that all children in the region have equal access to excellent care. 

As a patient or a parent of a child/young person on Long Term Ventilation (LTV) we would be very grateful for your help. This questionnaire will enable the STPN to gather feedback on the discharge process for LTV patients. Please could you take a few minutes to complete this anonymised questionnaire.
